Will speaking during massage affect the process in any way?

Massage therapy is more beneficial if done in quiet sessions. Speaking during massage sessions won't help you to experience the relaxation to its fullest extent. Most massage therapists won't engage in conversations during sessions. However if you wish to converse with your therapist, you can always do so. It's entirely up to you to choose whether or not to have a quiet session.

Is it mandatory to fill out forms relating to health history?

A therapist would like to know about your health situation before proceeding on with the therapy. This will be useful in planning and performing appropriate sessions for your body based on its needs.
